12,500 citizens as well as Nicaraguan refugees seek homestay in Alajuelita, Costa Rica, which is just outside of the nation's capital city, San Jose. A lack of economic opportunity and political unrest has lead to the settlement of Nicaraguan refugees, made up primarily of women and children, who are not eligible for health insurance because they are not citizens.

The Foundation of International Medical Relief of Women and Children (FIMRC), implemented their model clinic in Alajuelita, Costa Rica in 2004. Since then, FIMRC has become a central safe-haven for the vast number of uninsured individuals in this impoverished section of the country.
